Abstract

The research focused on the process of implementing a practice education model of Nursing Students (NS) in Clinical Practice (CP), called Service-School, based on the experiences of the implementation of the Dedicated Education Unit, and that was implemented by the Institute of Health Sciences – Porto of the Catholic University of Portugal, in one of the partner hospitals. The choice of the theme was related to the importance of the CP on the nursing graduation course to acquire the skills needed for the profession and with the changes arising from de integration of nursing in higher education. The purpose of the study was to know how they can transform an existing health care unit in a unit dedicated to the nursing education. The study refers to the period between September 2009 and September 2011, consisting of a descriptive exploratory study using the method of case study of a single case with multiple units of analysis. Initially, made up exploratory analysis of the protocol document collaboration and CP documents, non-participant informal observation of institutional meetings and key informant interviews. In the second phase took place non-participant observation in 5 units of the hospital where took place the CP concerned, interviews with head nurses and teachers involved, launching online questionnaire to nurses, content analysis of critical reflections and units evaluations performed by the NS and descriptive statistical analysis of the marks obtained by the NS in the CP. The results showed: (i) the positive contribution of the presence of the NS for the nursing team and to overall improvement care; (ii) improvement on the preparation of mentors and their performance; and (iii) relative approximation mentor/teacher and unit/school. Also revealed: (i) institutional communication gap; (ii) gaps in the selection of the mentors; (iii) overhead of mentors when inserted in the work plan with discrepancies and inadequate ratios mentor/patients and mentor/NS and decreased time for supervisory activities; (iv) lack of involvement of the nurses in the clinical education; (v) multiple discrepancies theory/practice, with high concentration on the technical aspect and less on the patient overall approach, as advocated by school; and (vi) dynamic of more targeted units to the institutional goals and less to the learning needs of the NS. The implementation of the model was compromised because of the inter- institutional barriers encountered, also identified on other studies, with little recognition and prioritization of issues related with education by the hospital. Revealed the need for: (i) greater proximity and involvement of both institutions and their stakeholders; (ii) the adequacy of staffing (nurses and teachers); (iii) clarification of the model; and (iv) convergence of the institutional goals as the education, nursing care and research. We conclude that de unit dedicated to the nursing education requires prioritization of the education on a continuous and systematic, and its professionals, a permanent attitude of openness and innovation, questioning their practices and with the partnership with the school, are able to identify strategies to improve teaching and care. We propose the creation of a group monitoring the implementation process, development of a protocol implementation, increased preparation of nurses and spaces of joint education (teachers and nurses), and analysis of the teacher’s role as a link between the school and the health unit.
