Introduction: Periodontal plastic surgery has improved tooth function and esthetics. The root coverage surgeries associated with the coronally advanced flap technique have shown a high predictability in the treatment of Miller's class I and II gingival recessions. The coronally advanced flap technique associated with enamel matrix proteins presents a different approach with potential advantages in optimizing the final aesthetic result in multiple gingival recessions. Objective: The aim of this study is to evaluate the efficacy of enamel matrix derivative (Emdogain®) in the coronally advanced flap technique of Miller's class I and II gingival recessions. Materials and Methods: A literature review of published English-language studies was conducted between 2000 and 2017 in two international MEDLINE (PubMed) and Cochrane Library databases, with articles selected using various Boolean descriptors and operators and using two reviewers who evaluated the quality of the methodological studies. Inclusion criteria: Articles of randomized controlled clinical trials in which gingival recession treatment was performed using the coronally advanced flap technique associated with enamel matrix derivative (EMD), with a follow-up greater or equal to 6 months, in patients with Miller class I or II gingival recessions in split mouth studies. Results: Recovering the gingival recession using the coronally advanced flap technique associated with enamel matrix derivative presented the best at the level of clinical insertion (CAL) and recession reduction (RD). Conclusion: The results of clinical reference parameters in the treatment of gingival recessions are better in patients submitted to the coronally advanced flap technique associated to enamel matrix derivative, Emdogain® (CAF + EMD) than in patients only subjected to the technique of coronal advancement flap (CAF), statistically significant at the insertion level (CAL) and keratinized tissue (KT).
