Today, technology is ubiquitous and an integral part of everyday life. In the case of healthcare services, digital transformation brings with it many challenges. The process of introducing new technologies can be quite lengthy and complex, affecting the healthcare system. The relationship between doctors and patients is put to the test and the delivery of healthcare services is fundamentally altered. Thus, this work, which had the Hospital Conde Ferreira as the internship site, aims to identify the key concepts that underlie the development of a new revolution in health care, Health 4.0, exploring the various elements and the importance of technology in this new digital era to comment on empirical evidence from a better understanding of the aspects of change and renewal for digital value chains in health. The rationale of this study was to support the Hospital's ambition to create a new support service to the Alzheimer's Day Hospital, an ambitious service that intends those patients in early stages of the disease do not have to give up their daily lives and can benefit from some medical services usually provided in the hospital from the comfort of their homes. An overview of health in Portugal and the current situation of implementation of telemedicine measures is outlined, identifying opportunities and constraints arising from it, to conclude the main ideas so that the Hospital Conde Ferreira can benefit from the implementation of the new service, elaborate some strategic changes and be able to enable the organization to structural and cultural changes.
