The present work is part of the Curricular Unit Final Stage and Report, inserted in the curricular plan of the Master's Course in Nursing with Specialization in Community Nursing, in the area of Community Health and Public Health Nursing, of the Institute of Health Sciences of the Universidade Católica do Porto. Its main purpose is to describe and analyze the experiences lived during the referred stage. Community empowerment was chosen as the central issue for the epidemiological surveillance of nursing diagnoses, with the Public Health Unit as the epicenter and a Group of Health Centers nurses as a target of care. Based on the analysis of the Local Health Diagnosis and the Local Health Plan, of the Group of Health Centers where the internship took place, the priority nursing phenomena for potential epidemiological surveillance were identified and the documentation rates of patients were analyzed same by the nurses. At the same time, the level of nurses' community empowerment for epidemiological surveillance of nursing diagnoses was assessed. As a methodology, Health Planning was used, adapting it to the clinical decision process of the Specialist Nurse in Community Health and Public Health Nursing, based on the Evaluation, Intervention and Community Empowerment Model. This model promotes community empowerment as a process and as a result of nurses' interventions and has a clinical decision matrix that guides their approach, from the diagnostic activity to the evaluation of results. In the diagnosis of the situation, a low level of community empowerment was identified for the epidemiological surveillance of nursing diagnoses, as well as the nursing diagnosis of committed community management, with the compromise of all its dimensions: community leadership, community participation and the community process. Following the stages of Health Planning, priorities were determined, objectives were set, certain strategies and interventions were taken, with a view to obtaining gains in the level of community empowerment. This internship was a relevant component in the continuous training process, allowing the construction of a body of knowledge essential to the exercise of the profession. In addition, it contributed significantly to the acquisition of common and specific skills of the Specialist Nurse in Community Nursing, as well as Master of Nursing. It concludes by stating that the way of understanding and exercising the assigned functions, in the context of primary health care, has undergone changes as a result of the competences developed throughout the course in general, and the internship in particular, being the deepening knowledge and skills development, indispensable to the discipline and the nursing profession.
