Abstract

Health vulnerability exists when well-being is not achieved throughout the child's life cycle, and it is up to nurses to work on projects to achieve the best possible levels of health. In order to better understand the concept of health vulnerability and how nurses can act on it, a Scoping Review was carried out on the theme "The concept of health vulnerability in the pediatric population in the context of nursing care". The results obtained were related to the health diagnoses with the greatest risk of vulnerability, belonging to a minority, health risk habits and the impact of the community and the surrounding context on the health of children and young people. The theoretical framework was drawn up mainly taking into account Nola Pender's theoretical framework, the Health Promotion Model, based on a vision guided by the philosophy of family-centered care, care partnership and atraumatic care, referenced by the College of Child Health and Pediatric Nursing Specialty. An internship was carried out in four clinical contexts, where projects were developed taking into account their respective characteristics and, above all, the target population of care, children, young people and their families who were experiencing situations of health vulnerability. Objectives were outlined that could be achieved through the implementation of activities, with a focus on promoting the common and specific skills of the Specialist Nurse in Child and Pediatric Health Nursing. Particularly, the activities carried out at the Family Health Unit, where follow-up consultations were held to monitor health gains for children and young people and their families. In the Pediatric Day Hospital, a Checklist was drawn up for the Transition Process from the Pediatric Service to the Adult Service, which will later become a standard hospital procedure. In the Neonatology Service, a culturally congruent health promotion project was carried out, aimed at newborns and their immigrant families, using the "Immigrant Family Welcome Box". Finally, in the Pediatric Emergency Department, a project was carried out to raise awareness of the abuse of children and young people, through training sessions aimed at the nursing team. Throughout the various contexts, the main goal was to act to reduce the vulnerability identified with health-promoting nursing interventions for children, young people and their families, ensuring the involvement of the nursing team as a whole in order to maintain the projects at the end of the internship.
