Abstract

This report presents the activities throughout the probationary period relating to the module I and II. The Module I was based on the first and second phase of the Health Planning, which refers to the Diagnosis of Health Situation of the population with Diabetes Mellitus and Prioritization of Needs. In Module II with the Intervention projects in the needs previously detected and the evaluation of them, it´s possible remark the gains in health . Initially this report observed all health diagnosis developed in three groups : users Diabetics and Non- Diabetics Grouping Health Centre ( ACES ) - Eastern Porto which are institutionalized in Homes and Day Centres and Diabetics users from the Family Health Unit ( FHU) Faria Guimarães . My area of community intervention was extended, however in consensus with experts in the area directed to activities to diabetic users enrolled in USF Faria Guimarães as the time for holding stage would not allow us to intervene elsewhere. Internship in Module I proceeded to collect data through the application of a form already built by the Portuguese Catholic University ( UCP ) - Institute of Health Sciences ( ICS ) , followed by the processing and analysis of data by the Statistical Programme Package for Social Sciences ( SPSS ) (version 19 ) , in order to identify the needs of the population . Be priorities established by the method of Halon, which were directed to the therapeutic regimen triad in diabetes. In Module II projects were implemented in order to meet the needs expressed in the triad of the management of the diabetic regimen, in the area of diet and exercise. Took place as sessions action strategies for health education about healthy eating and exercise, and in partnership with fellow stage commemorated the World Diabetes Day on 14 November. Having the vaccination has emerged as a felt need, taking the contacts from the other actions and take advantage of every moment of AEPS and update vaccination schemes that were outdated. The field experience proved rewarding , because working with the community in loco , assign an image of the nursing profession of great value and benefit to the community.
