Introduction: Clinical Decision Support Systems (CDSS) are computer software, developed to provide specific support for health professionals in making clinical decisions. In Dentistry, radiographic images are important auxiliary means to aid in correct diagnosis. The software ORAD III is a CDSS developed to evaluate the clinical and radiographic characteristics of intraosseous lesions of oral cavity in order to assist in clinical diagnosis. Objective: To evaluate the reliability of the CDSS, in patients who underwent biopsy of the hard tissues of the oral cavity. Material and Methods: A total of 38 radiographic images from patients of the dental clinic of the UCP and the Public Hospital of Zagreb were subsequently analysed by 10 students of the 5th year and 10 students of the 4th year of Dentistry. The control group was consisted of 6 specialists in Medicine and Oral Pathology area. The parameters of each lesions were introduced in the computer program ORAD III, which gave a list of possible diagnoses (from most to least likely). All the observed pathologies were previously diagnosed by biopsy and histopathological confirmation, not known by the operators. It was developed tables comparing both histopathologic diagnosis and diagnosis given by the operators, without the use ORAD III program. Later, it was proceeded to compare the diagnoses obtained through the CDCC with the histopathological results in terms of success rates (1st, 2nd or 3rd diagnostic option ORAD III) and failure. Results: At the first moment, there was no significant results (p=0.069). However, at second moment both tests (1st diagnostic option and sum of three diagnostic options) had statistically significant results (p=0.049 and p=0.011 respectively). In both cases control group had best results (26.92% e 50.00%, respectively) Discussion: Lack of clinical data was relevant to this study. Knowledge and level of lesion’s characterization is the major factor in the use of ORAD III; Conclusion: Within the limitation of our study, it was possible to draw conclusions with statistical significance that demonstrate the potential of ORAD
