Abstract

Palliative Care are active and total care, provided by professionals with specific training and proper training, requiring technical preparation, theoretical and practical experience. A differentiated training in palliative care is essential and indispensable to the organization and quality of care. In the context of the Master in Palliative Care of Portuguese Catholic University, devocationalise, was realised an internship of 300 contact hours in Palliative Care Unit of the Hospital da Luz and made this report. The overall objective of the internship was to acquire expertise in 4 key areas of palliative care: Teamwork, Symptom Management, Adequate Communication and Family Support. As a nurse with advanced training in palliative care, developed skills through training, analysis and reflection, in order to achieve an optimum outcome for assistance, conducting an ongoing evaluation, global and holistic of the patient's health and family, resolving complex problems based on scientific evidence. With regard to Teamwork, my goal was to integrate the interdisciplinary team and learn strategies for team work. To do so, I participated actively in the care team meetings and gave my contribution as a nurse with advanced training in palliative care. In the Symptom Management, I used the skills I gained through the needs assessment, monitoring of symptoms, pharmacological therapy and nonpharmacological, reflecting on the benefit to the patient. Adequate Communication skills were developed through reflection and analysis meetings help, whose aim was the establishment of the help for the relief of suffering. As for Family Support, I emphasized the importance of family inclusion in care plan, approaching the support in bereavement. Highlight the family conference as a key instrument in supporting the family, describing and reflecting on the conference I attended. I developed pedagogic skills and scientific, by conducting training and research in this area, once this need was detected by the team, contributing to continuous quality improvement. The last chapter includes the Case Study, where I realized an individual plan of intervention to the problems identified in the assessment, by implementing therapeutic strategies discussed with the patient / family and team, with benefit to the patient himself and his family. This case study enabled me to solidify skills in the four key areas. In addition to describing the activities to achieve the proposed objectives, I explained the full range of knowledge acquired through observation, training and research in achieving this report, making it richer. I feel really competent "to improve quality of life of patients with problems arising from an incurable disease with limited prognosis, and / or serious illness (life threatening), and their families through prevention and relief of suffering, using the early identification, appropriate assessment and rigorous treatment of not only physical problems, such as pain, but also psycho social and spiritual " (1), that is, in providing skilled palliative care.
