Abstract

This document is produced during the course "Final Traineeship and Report" within the scope of the Master's Degree in Instituto das Ciências da Saúde (Porto) of the Universidade Católica Portuguesa. This report - "Construction of a Specialised Personal and Professional Identity" aims to highlight the acquisition of common and specific skills of the nurse specialist in Medical-Surgical Nursing during the internship in a pre-hospital care context carried out at the National Institute of Medical Emergency (North) and in an intensive care context at the Multipurpose Intensive Care Unit of a hospital in the northern region of Portugal.The report is based on a descriptive and reflective methodology on the experiences lived in these two internship contexts and is structured based on the different domains of the common competencies of specialist nurses and the specific competencies of the master's degree area. During the internship period, training needs were identified in both contexts and strategies were developed to meet those needs with the purpose of making my presence and the knowledge brought to the services fruitful. Thus, in the first context, pre-hospital care, a literature review entitled "Tranexamic acid in the context of pre-hospital trauma" was carried out, colliding with a change to the National Institute of Medical Emergency haemorrhage protocol. In the second context, Multipurpose Intensive Care Unit, a guidebook for the provision of nursing care to neurocritical patients called "Neurocritical Patient" was developed. Also, during the internship, a scientific article entitled "Social Isolation by SARS-CoV2: Needs and strategies" was developed in partnership with another student of the Master's Degree in Medical-Surgical Nursing.In addition to the contributions towards service improvement, the research and production of the above mentioned documents, together with all the work developed during the internship, allowed for the development of common competencies of specialist nurses, namely the development of a professional, ethical and legal practice in the area of specialisation in Medical-Surgical Nursing, the assurance of a dynamic role in the development and support of institutional strategic initiatives in the area of clinical governance, developing skills in nursing care management, optimising the team's response and articulation within the health team and adapting leadership and resource management to situations and context, aiming at ensuring the quality of care and developing self-knowledge and assertiveness, basing specialised clinical praxis on scientific evidence.In addition, it was also possible to acquire specific skills of the nurse specialist in medical-surgical nursing, namely skills related to the provision of care to the person, family/caregiver experiencing complex processes of critical illness and/or organ failure, in the dynamization of the response in emergency, exceptional and catastrophic situations, and also in maximizing the prevention, intervention and control of infection and antimicrobial resistance in critically ill patients and/or organ failure, given the complexity of the situation and the need for timely and appropriate responses.
