Abstract

Pediatric care should be provided at home or on a daily basis, whenever that is possible. Thus, this internship report provides a reflective analysis on the acquisition and development of the common competences of a Master in Nursing, specializing in Child and Paediatric Health, with theoretical support from Afaf Meleis' Transitions Theory and the Paediatric Home Hospitalization Care Model. Paediatric Home Hospitalization promotes family union and comfort, providing a more humanized experience compared to conventional hospitalization. Therefore, nursing care during this period was provided in an individualized and personalized way, with a focus on the transition to home, through parental support and training to enable a timely but safe discharge process for all age groups. With the general objective of "developing common and specific Specialist Nurse in Child and Paediatric Health Nursing competencies in the provision of specialized care to children/young people and their families, with an emphasis on the hospital-home transition", specialized interventions were developed in the context of neonatology, paediatric intensive care and paediatric inpatient care, respecting the Project Work Methodology. In line with the objectives outlined in each internship context, some of the interventions developed stand out. In the neonatology unit, family-centred care was provided to promote safe and empowered transition to home, through parent groups with training component, family follow-up and optimization of the nursing transfer/discharge letter. In the intensive care unit, emphasis was placed on assessing parental satisfaction as a means of continuous improvement and standardizing care for children with diabetes mellitus. Finally, in the inpatient unit, a plan was drawn up to promote an effective transition to home for children/young people with tuberculosis, creating a therapeutic board game and a quiz that allows association between training with play. Also noteworthy is the submission for publication of a systematic review of qualitative studies and other scientific dissemination work in the field of Paediatric Home Hospitalization and continuous improvement of care. This entire journey has therefore resulted in the acquisition and development of Master's competences, common to Specialist Nurses and specific to Specialist Nurses in Child and Paediatric Health.
