Abstract

The present report is part of the studies to acquire a Masters Degree in Nursing in the Medical-Surgical Field, Caring for a Critical Patient from The Institute of Science and Health from the Portuguese Catholic University in Lisbon. During the elaboration of this report Regulation nº 140/2019 “Regulation of Common Competencies from the Nurse Specialist”, regulation nº 429/2018 “Regulation of the Common Competencies of the Nurse Specialist in the Medical-Surgical Field, Caring for the Critical Patient” were used as support in a descriptive and reflexive analysis based on the Self-care Deficit Nursing Theory from Dorothea Orem. The idea behind this project was to develop a rigorous and specialized practice, providing adequate and safe care to critical patients based on reliable and recent evidence. The main objectives of this project were to develop competencies in the nursing care to the Neurocritical Patient and for Patient with Acute Kidney Injury who needed Continuous Renal Replacement Therapy. This project was implemented in an Intensive Care Unit of a Central Hospital in the Lisbon area and in a Hemodialyses Unit in the periphery of the Lisbon area, this report was written to show the conclusions. The clinical report is a reflection of the professional progression. The main objective was to show the acquisition and development of new competencies in the nursing care plan for critical patient. Throughout this project, a Systematic Review of Literature in order to map the Nurse Care Plan for the Critical Patient with Acute Kidney Injury under Continuous Renal Replacement Therapy. In the course of my professional development, in the Intensive Care Unit, I created a guidance document on principles and modalities of renal replacement therapy and nursing care plan for the Patient with acute kidney injury under continuous renal replacement therapy. Here, I also developed a support guide for the application of an assessment tool for the prevention and detection of Delirium. At the Dialysis Unit, I realized a leaflet entitled "How to care for an Arteriovenous Access Care BEFORE and AFTER Treatment" for patient to answer frequently asked questions and doubts. I also wrote an action and training plan entitled "Clinical Monitoring of the Arteriovenous Access Cannulation Technique" for the nursing team.
