Abstract

Scientific and social advances that took place in the twentieth century led to an increase in longevity, resulting in an increase of chronic non-communicable diseases. When death is seen as an health care failure there are no concern about the patient quality of life. Palliative care affirms life and regards dying as normal process and is principles are known. However, this philosophy of care has not spread equally all over the world. The European Union considers the access to palliative care as an human right.Palliative care interdisciplinary teams sustain patient and family (or carer) satisfaction with the care given, either in the hospital setting as in the community.National Health Service considers the community as the most important setting in order to promote palliative care and defends a more structured organization because most of the patients to stay at home when they are dying.According to such an established and priority given health diagnosis performed for users /carers covered by the health teams of Group Health Centre Maia and their health professionals were developed intervention projects. One project headed toward the promotion of quality of lifeof patients or caregivers in order to meet the needs identified, which appear to be a requirementfor assistance from the Palliative Care in the community.One project headed toward the promotion of quality of life of patients or caregivers in orderto meet the needs identified, which appear to be a requirement for assistance from the Palliative Care in the community. The family and other caregivers, as part of a user as they correspond tosignificant emotional people, have a vital role in supporting users with advanced and progressive disease in end stage suffering from the impact. Family members are carers but they also need professional care.Palliative care meet the cross effects that health care can generate in the people whosurround the user, being essential to target health professionals and technicians to include the family in the care plan drawn, often assessing their level of stress and identifying problems thatcan be solved by interacting with all available resources, both with health professionals or otherresources identified as necessary for such a situation.For such action is essential to develop education and promote specialized training toprofessionals from Primary Care in relation to key areas of palliative care. Consequently there was a project to promote and implement such quality care it appears to be essential to value the symptoms manifested, the problems of users / carers as well as the appropriate intervention ineach case, requiring that health professionals acquire proper training and rigorous in the key areas of that care.
