This report developed under the Master's Degree in Nursing with specialization in Community Nursing seeks to describe the three stages of modules which took place in the Community Care Unit - Nostra Pontinha, Pools Loures-Odivelas Health Centers. According to which happened in the last years, the cardiovascular brain diseases remain the leading cause of death at national and European level. Therefore it leads to serious consequences for the society and the health system, determining that they should be regarded as one of the most significant public health problems in urgent need of ease. Therefore, the health promotion is becoming essential for the development of personal and social skills through education for health seeking to increase the power of the communities, empowering them. Thus, the subject covered in this study was the Prevention of Cardiovascular Brain Diseases in order to promote health and prevent disease by adopting healthier lifestyles. During Module I, a health diagnosis was made to the working class that integrates the main community resources of neighborhood Mário Madeira - Pontinha, it was performed a descriptive study, through the application of a form, as an instrument of data collection. The resources mentioned above refer to the Schools (JI Gil Eanes and EB Dr. Mário Madeira), Day care of the Holy Family and the Public Security Police, yielding a sample of 100% in the first two community resources and 40% in the last one. Data analysis was performed using the quantitative methodology. Lifestyles, risk factors and family historical of brain-cardiovascular disease were evaluated, whose results were higher in the following cases: overweight individuals, increased abdominal size, lack of physical exercise, bad eating habits, the risk of developing diabetes and hereditary diseases. Taking into account the needs and obtained results, prioritizing them, in Stage II Module has been made the Intervention Project, entitled "Act to Prevent", organized in three parts, "Healthy Eating Habits – soup? Yes, thank you"; "Training Basic Life Support" and "Panel / group dynamics", planned in two phases, phase 1 - module II and phase 2 - module III. Several activities were developed such as education activities for health promotion and community involvement. The second phase of the project was to continue the strengthening and the promotion of the community intervention. Throughout these modules were developed many other activities in many different community areas, provided by ongoing projects in the UCC, contributing to the acquisition, coordination and skills development focused on specialized nursing care to individuals, families and communities.
