Introduction: The craniofacial growth pattern can predispose to Obstructive Sleep Apnea (OSA), due to the reduction of the upper airway which makes breathing difficult. In addition to limited space in the oral cavity for proper tongue positioning, this contributes to inadequate craniofacial growth. The close relationship between the oral cavity and the respiratory system and these anatomical features make it possible to identify clinical signs, such as snoring on waking, and favor nocturnal mouth breathing over nasal breathing. Objective: Analyze current knowledge and literature on the effects of Twin-Block on the upper airway (UA) in apneic pediatric patients and on the correction of mandibular retrognatism, with consequent improvement in the symptoms of OSA and quality of life. Material and Methods: Systematic literature review according to the Joanna Briggs Institute (JBI) guidelines, using the Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) methodology. A systematic review was carried out in the PubMed/Medline, Web of Science and Scopus databases. The selection of studies was made by two reviewers who carried out the inclusion/exclusion of studies according to the previously defined eligibility criteria. Results: The six studies included in this systematic literature review suggest that the Twin-Block appliance can improve the patient's facial profile and OSA symptoms in carefully selected children who presented with OSA and mandibular retrognathia. As the anteroposterior relationship between the maxilla and mandible improves, the tongue and hyoid bone move forward, which seems to increase the volume of the VAS and should be gradually considered by dentists. Conclusion: The present systematic review demonstrated evidence of improvement in the signs and symptoms of OSA and quality of life in pediatric apneic patients after treatment with Twin-Block for correction of skeletal class II and mandibular retrognathia.
