Abstract

According to estimates by the World Health Organization, in global terms the number of people over 65 years old could reach 22% in 2050 and reach a value close to 34% in Europe, which will then mean that one in three Europeans will have 65 years of age or older (Fonseca, 2018). Epidemiological data reveal that population aging has been increasing gradually over the years, maintaining an increasing trend. However, the impact of population aging on society will depend, in part, on the nature of the policies that will respond to this new reality. Living longer also means being more exposed to risks, such as vulnerability of health status, social isolation and loneliness, violence and dependence. In this sense, it is essential to implement social and health policies that improve the quality of life of the elderly. The Linha 65 Project has the elderly and/or dependent people as its target population, in a context of community health. The Community Care Unit, where the Final Internship was carried out within the scope of the Master's in Community Health and Public Health Nursing, fits the Project in a partnership action, focusing on health care within the scope of primary prevention. In the context of the Final Internship and Report, Curricular Unit of the aforementioned Master's, a Community Intervention Project called "Community Empowerment in Optimizing Nurse Intervention in the Line 65 Project - Community Nursing Intervention" was developed, which had as its target population the nurses who intervene in the Linha 65 Project, both those from UCC S and those of the Family Health Units and the Personalized Health Care Units, with the general objective of optimizing the intervention of nurses in the Linha 65 Project. The project used the Health Planning methodology according to the author António Tavares (1990) and as a theoretical reference model the Assessment, Intervention and Community Empowerment Model by Pedro Melo (2020). The course taken during the Final Stage and this Report made it possible to acquire skills, namely the Common Skills of the Specialist Nurse, the Specific Skills of the Specialist Nurse in Community Health and Public Health Nursing and the Master's Skills, a central objective for obtaining a Master's degree in Nursing, with a specialization in Community Nursing: Area of Community Health and Public Health Nursing.
