This summary describes the course taken along the traineeship, in order to obtain skillsin the area of Specialization in Community Nursing. The traineeship developed in twoUnits (Module I-Health assessment and Module II- Intervention according needs) thattook place between April the 20th and June the 20th 2009, and October the 6th andNovember the 27th 2009, respectively, in the Vila Real I Health Care Centre.Activities in different fields of the Community Nursing have been developed, reflectingthe diversity and multiplicity of areas in which there was a need to deepen knowledgeand skills. In the context of Unit I, a health diagnose was made restrict to theAdherence/Management of the Therapeutic Regimen and Self Care in Diabetes.Priorities have been defined, emerging as intervention area the promotion of physicalactivity in people with Diabetes type 2. In Unit II, an intervention project was conceived(Pro Active: Educational Project for People with diabetes type 2), executed andevaluated, and the I Diabetes Fair took place.The Pro Active project: Educational Project for People with diabetes type 2, tried topromote an informed choice and to develop skills and competences in the managementof health/sickness processes, through sessions of Education for Health, based on theconstitution of an operative group.The First Diabetes Fair aimed to promote healthy life styles, relevant information forthe accession to salutary behaviours, screening of risk factors to Diabetes, screening ofthe diabetic foot, accession to the therapeutic regimen in Diabetes, and distribution ofspecific materials to Self Care.Other activities have been developed in order to evolve in other competences in the areaof the specialization in Community Nursing, namely in the context of School Health,Local Coordinating Team of the National Network for the Continuous and IntegratedCare and Preventive Measures to H1N1 virus, based on Community Empowerment.
