Introduction: The present systematic review followed the methodology defined by (PRISMA), following the PICO criteria. The following question was established: Does the presence of SNPs influence the susceptibility of patients undergoing dental implant placement to develop peri-implantitis? The bibliographic search was carried out in the Medline/PubMed, Web of Science and Scopus databases using the search expression: "(Polymorphism, Single Nucleotide) OR (SNPs)) AND (Peri-implantitis)". After the search, nineteen articles were selected for the systematic review. Objective: To identify the SNPs with demonstrated association with the incidence of peri-implantitis, in order to systematize the existing evidence on the genetic factors underlying this condition. Results: In the nineteen studies, a total of 23 genes and 32 SNPs were evaluated for a possible relationship with the occurrence of peri-implantitis. Thirteen of the 32 SNPs analyzed showed significant differences in the distribution of allelic or genotypic frequencies. The IL1B rs1143634 polymorphism was evaluated in four studies, revealing significant differences in the distribution of allele frequencies between the case and control groups. The TNF rs1800629 polymorphism was evaluated in five studies; carriers of the variant A allele have a significantly higher risk of developing peri-implantitis. The CD14 rs2569190 polymorphism was evaluated in two studies, although neither reported significant differences in the distribution of allele and genotype frequencies. The OPG rs2073618 polymorphism was evaluated in two studies, and significant differences were observed in the distribution of allele frequencies. Conclusion: Sixteen polymorphisms were found to be significantly associated with peri-implantitis. Seven of these - CD14 rs2569190, FCGR2B rs1050501, FCGR2A rs1801274, FCGR3A rs396991, IL10 rs1800872, IL1B rs1143634 and TNF rs1800629 - showed significant associations in the 3 transmission models considered in the analysis. Four polymorphisms showed significant concordant results in two or more independent studies: IL1B rs1143634, TNF rs1800629, CD14 rs2569190 and OPG rs2073618.
