Abstract

The progressive aging of the population and the consequent increase in chronic diseases, imply a substantial consumption of drugs. Given the vulnerability of population segments with older ages, proved to be pertinent to study the issue of Drug Regimen in Elderly. The stage Module I in Continuing Care Unit (CCU) of Campanhã (ACES Eastern Harbour) focused on conducting a health diagnosis of the population aged 65 years or more, focusing on the program area of Drug Administration. In this context, we performed the collection, analysis and interpretation of data from a random sample of users. The study revealed that the sample (100 elderly) whose main problems the economic situation (30% of the sample referred difficulty having money to buy medication), decreased cognition (15% of the sample showing difficulty reading the packaging) and the loss of mobility (14% of the sample referred trouble moving up to the pharmacy). The data suggest that should reduce the complexity of the drug regimen (average of drugs found was 5.7 ± 2.8), so they were designated as the needs Accession and the Medication Management System. The identification of needs allowed to define proximity intervention strategies through domiciliary visitations. In stage Module II were outlined individual intervention plans for a small group of users referenced by UCC Campanhã, with the diagnosis of ineffective management of Drug Administration. After documented nursing intervention (SESI, telephone interviews and home visitations), there were total or partial gains in health, depending on the user in question. There were total health gains to 25% of the selected sample and partial in 50%. Note that for the remaining 25% at the time of intervention, was not identified any need for intervention. In the last module the main objectives reported to know the reality of ECCI Campanhã, integrating domiciliary visitations, and to monitor the nursing consultations at the level of UCSP of São Roque da Lameira. We also carried out a research study on the elderly and dependency in self-care, with the creation of a data collection instrument for assessment of their addiction. The theme approach describes the assessment tools (indicators) dependency in self-care and thus enables future Health Situation Diagnostics in this particular program area. The internship experience corroborated the idea that it is in the community that the nurse has an important role as a driver of health education agent, taking into account the context of the individual and the health gains that he may realize.
