Abstract

Diabetes Mellitus affects about 13.6% of the portuguese adult population, being responsible for huge health expenditures in emergency resources and hospitalization days, being inserted in the context of the public health problem of non-communicable diseases. The therapeutic management of diabetes implies action in terms of glycemic control, diet, exercise regimen, management of drug regimen and foot surveillance, being very dependent on the patient, his ability to manage and his willingness to adhere to the treatment. therapeutic regimen. This report is part of the curricular unit Final Internship and Report of the 14th Master's Specialization in Community Nursing and Public Health (MEECSP), carried out in a Community Care Unit (UCC) in the northern region. The methodology of health planning is used to give shape to a diagnosis of the situation, with a community of diabetic people, with difficulties in ensuring the effective management of their therapeutic regimen, followed by a process of prioritization of needs. Knowledge, capacity, volition and resources were identified as problems in the study, in relation to glycemic assessment, food management, physical exercise, management of the medication regimen and in the performance of foot care. The main results obtained showed that the population has the knowledge, mostly has the capacity and resources, but does not have the volition to manage it. The committed volition for the management of the therapeutic regimen in Diabetes Mellitus was then defined as the main area of community intervention, and a diagnostic decision tree was elaborated to support the nursing intervention, combined with a data collection instrument, and carried out an intervention project, accompanied by process and result indicators sensitive to nursing care.
