Hypertension is a public health issue, both national and internationally. Therefore, it was the theme of the first module that took place at the “Unidade de Saúde Familiar Conde de Oeiras (USFCO)”, where a diagnosis was performed regarding the situation of the hypertensive patients of this unit. The hypertension thematic fulfils the needs felt by the team, which, having only been created a year earlier, mentioned that these patients were under-studied. This together with the fact that hypertension is one of the main risk factors of cardiovascular diseases, and the main cause for morbidity, disability and death in Portugal warrants the importance of this problematic (Marques da Silva, 2008). From the numerous results obtained from the situation diagnosis, the priority defined was nourishing, namely the consumption of salt and saturated fats, as well as learning how to read labels of food products. In this sense, in the second module, it was implemented an intervention project designated “Coma com HiperAtensão”, which acted at the three levels of prevention. This project was extended to the community through a partnership with the “Unidade de Cuidados na Comunidade Saúdar (UCC Saúdar)”, by promoting the enablement of patients to control the food they ingest. The third module, which was optional, took place at the “UCC Saúdar”, and was comprised of a reflexive analysis of the covered intervention areas. Associated with this reflection, it was established a partnership with the “Unidade de Saúde Familiar São Julião (USFSJ)” through a collaboration in the implementation of a project. The goal was to attain an awareness of the perception of patients, who are non-users of this unit, in order to promote their satisfaction, which constitutes a quality standard of the “Ordem dos Enfermeiros (OE)”. Both developed projects contribute to the corresponding units that promote the nursing care with greater extent and quality.
