Introduction: Endodontic treatment can be defined as an association between the chemical effects of irrigation, appropriate medication and, of course, mechanical instrumentation inside the root canals, being completed with a correct filling, with the purpose of preserving or reestablishing the root canal health of periradicular tissues. In clinical practice, the final diameter of the canal preparation will depend on the root volume, the presence of curvatures and the type of instrument used in the preparation, as well as the irrigants used and their activation. Objective: The purpose of this work was to carry out a systematic review to evaluate the effect of different apical calibers on canal disinfection. Materials and Methods: The search was carried out in the PubMed® and Web of Science® databases using specific terms related to the caliber of the apical preparation. The selection of studies was carried out by two independent researchers according to the inclusion and exclusion criteria. Results: Once the research and respective analysis were completed, 13 articles were included in this study. The final apical diameter should correspond to a file that has a large area of contact with the canal, taking care to respect the canal anatomy and without causing a risk of iatrogenesis. In order to allow the NiTi files to work with greater safety in the last apical millimeters, the middle and cervical thirds must be pre-widened, which will favor the reach of the auxiliary chemical substance with the aim of eliminating microorganisms, the chemical substance must always be activated, thus achieving success in endodontic treatment. Conclusion: More studies are needed to determinate the ideal apical caliber, however, through this systematic review we can conclude that very large preparations are not necessary for effective disinfection. Through the activation of irrigants and a good combination of them, we can achieve good canal disinfection, without changing the tooth structure too much.
