The disparate and multifaceted phenomena that plague today's society inexorably reiterate profound repercussions, with vulnerable populations being the most affected. Therefore, a structured and properly competent intervention is essential, and nurses must act in a community context in order to bring out the maximum health and well-being potential of people in vulnerable situations. In this regard, health literacy proves to be an unrivalled powerful tool for promoting health. In view of the above, a community intervention project was developed in a Community Care Unit, based on Nola Pender's Health Promotion Model and Imperatori & Giraldes's Health Planning Methodology. The theoretical-conceptual foundation was based on a literature review and consultation of structuring documents. The diagnosis of the situation focused on identifying the health needs of the target population and determining possible areas for community intervention using information obtained through a semi-structured interview with a key informant and a focus group with experts in the area of intervention with vulnerable populations. Considering the health problems and/or needs that emerged, priorities for action were defined. In this line of chain, objectives, targets and indicators were set, and strategies were stipulated through the elaboration of a scoping review. Therefore, the establishment of partnerships with local entities from a community point of view, the holding of health education sessions, and the development of information materials and dynamic documents stand out as key points for optimizing an effective response with a view to achieving health gains. The desired evaluation results were generally achieved satisfactorily, so it is possible to see an overall positive balance of the entire process. The learning process was the subject of a critical-reflective analysis that fosters the excellence of nursing as a discipline, science and profession; providing the development of master's competences, as well as common and specific competences of the specialist nurse, namely, in the field of Community Health Nursing and Public Health.
