The prevention and control of infection is a theme that generates some polemic because, even today this problems occurs throughout the world affecting since the more developed countries to the most needy of resources. To nurses, this matter is extremely important, since the nursing professional is linked directly and indirectly to the patient and everything around it. However, prevent and control the infection associated to the health care is the responsability of everyone! From the health professional to the patient. According to the Directorate General of Health Services after the application of "National Survey on Prevalence" in March 2009, to determine the prevalence of infections associated to the health care (IACS) and acquired infections in the community (IC), shows that the gross rate of prevalence of nosocomial infection was 9, 8%, being this rate similar to the ones submitted in recent studies carried in European countries, being however of the highest and according to the “National Plan for Prevention and Control of Infections Associated with Health Care” (PNCI)”, this shows that one of the strategies to reduce infections associated to health care (IACS) is to "promote and encourage the safe environment in the Health center and the good hygienisation practices”. In the 1st module of this report, i did the diagnosis of the situation in six “ACES Oeiras” health unit centers, with the support and collaboration of “CCI of ACES Oeiras”, where it was identified the problematic situation. Stands out as the main conclusion of this study that about 50% of the nurses wash their hands before contact with the patient, 90% of the nursing professionals don´t clean their hands before handling sterilized materials, as well as in most cases (76,6%) the recommended time to wash the hands is not complied. In the second module of this stage, address the needs felt by the CCI, was developed an outreach project called “Hands that Care” In the third and last module, an intervention project was implemented, sustained in primary prevention in order to control and minimize infections associated to the health care This project has contributed significantly to the “Infection Control Committee / ACES Oeiras/ Carnaxide” for the detection and intervention on the identified problem
