Abstract

The Medication Management System is a complex process involving the processes and support systems that are available to help people who self administer medication in the community for the benefit of better health, prescription drugs. This occurs after the medication is dispensed. It may involve from the support of informal carers, family members and health professionals or social action that one finds in the course of their care (Banning, 2007). The module I, refers to the Health Situation Diagnosis of the elderly population covered by UCC Campanhã to the program area of the therapeutic regimen management, more specifically, the Medication Regime. The process was essentially to activities that allowed highlight the needs of the elderly population: selection of a random sample of 100 individuals, development and application of a form and statistical processing of the information collected. The conclusion shows that this sample's main problems: the economic situation (30% report difficulty having money to buy medication), decreased cognition (15% reported having difficulty reading the packaging) and loss of mobility (14% reveals difficulty in moving to the pharmacy). The data also suggest that, should decrease the complexity of the drug regimen (average of drugs found was 5.7 ± 2.8), as such, needs met relate to the Accession and Medication Management System. In Module II, were previously referenced by UCC Campanhã four users with a diagnosis of ineffective Medication Management System, which fell the intervention itself and for which they were individual intervention plans set. Found the components for a Plan of Strategic Intervention: Sessions of Health Education Individual (SHEI) on the therapeutic regimen management, telephone interviews and home visitations, it was found that the results obtained in terms of health gains were positive, translating up these in total gains or partial depending on the target user intervention. The module III summarizes the collaboration in the ECCI Campanhã and UCSP of São Roque da Lameira. The experience has enabled the acquisition of skills and understand the surrounding dynamic, accompanying house calls and nursing visits. Were also carried out research work on the Care Provider Stress.
