Abstract

This report describes my performance in the different modules in the internship: Module II - Intensive Care that took place at Intermediate Care Unit of Medicine in Centro Hospitalar Vila Nova de Gaia from 19/04/2010 to 19/06/2010; Module III – Term Care that took place at Convalescent Unit of Hospital Nossa Senhora da Conceição de Valongo from 04/10/2010 to 27/11/2010 and later, in the same hospital, Module I – Emergency from 29/11/2010 to 29/01/2011. It was organized by the Domains of knowledge in Nursing and the objectives aimed to acquire competences in caring for the elderly and adults who develop a critical illness or organic failure. The four Domains of knowledge are the result of theoretical and practical knowledge. These are not isolated, so I mobilized and applied them simultaneously throughout the internship. The Scientific knowledge Domain results from the relation between concepts. In this domain I demonstrate an advanced knowledge in the specialization area, I’ve consulted literature and experts, found communication changes and put into practice the theoretical lessons. I’ve involved families in care. The Information Knowledge Domain results from observation, questioning, analysis and comparison. In this domain I’ve detected problem situations within the risk management, including prevention of infection and falls. I’ve reflected critically on the Manchester Triage, reminding the nurses about the autonomy and power of decision. The Nurse is responsible for functions delegation and needs to judge the ability to whom he will delegate. The Knowledge-be Domain refers to the nature of the person that I am, my values and how I get involved in the practice of nursing. I’ve tried to reduce stress and anxiety of families allowing visits beyond regular time. I became part of the nurse team, managed stress and followed emergency guidelines. I’ve created emergency guidelines manual and encouraged the emergency unit maintenance. The Practice Knowledge Domain is focused on execution. In the Convalescent Unit I’ve managed nurse care and acted effectively in pain or other needs of the patient / families. The caregiver role in the Convalescent Unit is one of the most importance and was improved by the formation that I accomplished. To manage quality I’ve developed Studies such as Antiseptics, Toners and Undertake formal or informal Training Sessions. The motivation, the easy integration and increased autonomy were factors that facilitate apprenticeship. Throughout this period I’ve acquired Specialist competencies. It permitted decision making through a critical reflective way that aloud assertiveness, and a higher degree of responsibility, I’ve mobilized knowledge and competencies to act holistically. All these competencies become to me Knowledge in Nursing, through the mobilization on this internship they became part of my professional practice. It was an enriching and unique experience
