Abstract

Diabetes Mellitus (DM) is a chronic disease on a large expansion in the world. In Portugal, it is estimated that about one third of its population has diabetes or is at risk.The Annual Report of the National Centre for Diabetes, 2009, said the area where youinsert the Tâmega ACES III - Sousa Valley North, recorded a prevalence of 13.9%, thehighest in continental Portugal. In the Diagnosis of Health Situation made to this community, it was found that one of the needs expressed was poor adherence to therapeutic regimen related to diet. In response to this need, the stage of Module II - Statement of Needs Change, held a community intervention, through the "Coma Bem,Viva Melhor!", using the Wheel of Food as a central figure, promoter a basic education.Diabetes is dependent variations of food, exercise, medication, stress and intercurrentillness, however, healthy diet is considered the cornerstone for maintaining the health ofthe patient. The pattern of diabetic food should be similar to the adult population ingeneral, adopting a balanced diet, varied and caloric suits your style of life andcomorbidities. Sessions were held for health education, an important workinginstrument for Nursing, a part educational, interactive and demonstrative to targetedgroups of diabetics enrolled in the Health Extension (ES) of S. Jorge Lowland. Theaction group has provided an educational environment, seeking self-care and thereforeadherence to treatment of this pathology. During this journey has been further developed as an activity within the Celebration of World Diabetes Day entitled "Coragem ... Abrace a Diabetes!”, targeting the general population, promoting aconscious reflection of the condition. The implementation of the education sessions forhealth, helped diabetics improve their lifestyle, promoted solutions to good control ofdiabetes through dietary teachings and covered 58.5% of clients with diabetes enrolledin the respective ES. These results were derived from a sense of inclusion and unity ofinterests motivating the participants, aiming at an average knowledge acquired in theorder of 46.9%, reflecting an indicator of results in 100%, resulting in health gains.
