Introduction: Absolute isolation with a rubber dam is an essential step regarding its advantages. It is considered a prerequisite when an endodontic treatment is required. In endodontics, in most treatments, only a single tooth is isolated and often it is noticed that the working field is not in the best position, a situation that reduces the efficiency of the rubber dam sheet and the working conditions, while increases the patient’s discomfort. Materials and methods: In this experimental study, 160 rubber dam markings were performed (80 using the Coltene Map and 80 using the New Marking System) which were later placed in the mouth, with clamps, in a total of 20 patients. Each patient received a total of 8 rubber dams (4 marked with the Map and 4 marked with the New Marking System) and measurements of horizontal deviation (in relation to the midline) and vertical deviation (in relation to the nose base) were made. The results were analyzed using SPSS® version 27. Results: The sample has a non-normal distribution (p < 0,05). Therefore, the Mann-Whitney U statistical test was applied. The measurements of the horizontal and vertical deviations of the markings that used the New Marking System are significantly lower in relation to the measurements of the markings that used the Map, also presented a smaller dispersion of results. Conclusions: The New Marking System showed better results in terms of positioning the operative field, which can improve the usability of absolute isolation and enhance its advantages.
