In a period where the resources devoted to healthcare are short and valuable, it is important to demonstrate which are the costs associated with the infection, so that one can understand the value on investing in prevention. The case of hip replacement surgery is the starting point. Methods: With this study we aim to contribute to the definition of an evaluation system of the social and economic impact of surgical site infection, have objectified the determination of: rate of surgical site infection; the average value of the infection; the social effect in patients with infection. We developed a retrospective case:control study in a population of 715 patients undergoing hip replacement surgery, between 2005 to 2009 , with a sample of 43 patients with infection. We used a form and a questionnaire (which includes assessment scale physical and mental health SF 36, validated to the Portuguese people by Ferreira (2000)). Results: A surgical site infection with a rate of 6,3 %, came mostly in women (74,4 %) with the average age of 78 years who underwent uncemented hip replacement due to fracture of the femoral neck (88,4 %). Compared with controls, patients with infection had more 12 days of hospitalization, used the operating room over 0,23 times, had a readmission rate of 13,9 % and consumed more antibiotics and exams. The patient with infection had an increased cost of € 6 375,97. In the social effect of the infection, the mortality rate in patients with infection was bigger, and they appear to have less physical health and lower emotional performance. Conclusion: The negative impact of infection after hip replacement surgery is real and we hope to sensitize managers and health professionals to the importance on investing in prevention
