Abstract

The present work intends to be the written expression of the interactive process covered in the development of specialized clinical skills proposed by the study plan of the Master of Enfermagem com Especialização em Enfermagem Médico-Cirúrgica developed by the Instituto de Ciências da Saúde da Universidade Católica Portuguesa, from the Course unit Report. The theoretical knowledge was complemented by a real practical component obtained during the internships : in the Domiciliary Hospitalization Unit and the Continued Care Unit, the Burnt Unit and the General Emergency Department (with an observation in the Emergency Medical Car).The theoretical model of the Afaf Meleis Transitions related to the health-disease process of the patient with wound is a transversal axis of report. The internships aim was specialized care to the critical patient and his family in different contexts and sought to promote specific skills in the area of the wound navigator. The report is based on the decision-making process relating to the interships. The developed themes describe critically the strategies undertaken to achieve the defined objectives and lists the skills developed.From the diverse range of activities performed during the interships the following standout: an integrative review of the literature on "The skills of the wound navigator in the health care team"; a record sheet for evaluation and monitoring of wound patients and the respective electronic database (in Microsoft Excel) for data treatment; support decision making tool to aid the choice of dressing material; a sectorial standard on the prevention of contamination and decontamination of the home visitation bag; in-service training on therapeutic options in the burnt, initial approach to burns and pressure ulcers; sectorial procedure rules on pressure ulcer registration method; poster of the world day of pressure ulcer prevention; and thematic dossiers.
