Percurso e desenvolvimento de competências específicas em enfermagem médico-cirúrgica na área da pessoa em situação crítica

Abstract

This report is a portrait of the path to acquiring skills in the Master's Degree in Medical-Surgical Nursing, in the area of Nursing for Critical ill patient, at the Nursing School (Porto) of the Universidade Católica Portuguesa, during the completion of the internships in the Cardiothoracic Surgery Intensive Care Unit and in the Polyvalent Emergency Service, in 2 Hospitals in the North of Portugal, during the period from September 4th, 2023 to December 16th, 2023. This document was prepared with the Competencies as a reference Common Standards for Specialist Nurses, proposed by the Order of Nurses, which is the basis for this report. Throughout the work, the development of activities and experiences in a clinical context are described, corroborating the most recent and relevant scientific evidence. These activities in both clinical practice contexts allowed the acquisition of specific skills in specialization of Medical-Surgical Nursing, in Person in Critical Situation. From the path developed for the acquisition and development of specialized skills in Nursing Care for Person in Critical Situation, the main activities carried out are described, highlighting in this report the literature reviews carried out within the scope of Infection Control Scales in the context of Intensive Medicine Service, in which there are assessment scales on the subject that can be applied by nurses; Pneumonia Associated with Intubation in the context of a Cardiothoracic Surgery Intensive Care Unit, which culminates in updating and raising awareness among nurses regarding the bundles proposed by the General Directorate of Health in 2022; and also the Sepsis’ Bundle in the Emergency Service, which results in the development of a questionnaire to assess the knowledge and difficulties experienced by nurses who perform screening in identifying and activating the Sepsis’ Bundle. Also, throughout the work, the main activities carried out and the skills acquired within the scope of Ethics and Deontology, Nursing Care Management and also the Guarantee of Continuous Quality Improvement, within the scope of Critical ill patient, will be described, comparing with practice clinic carried out with the most recent and relevant scientific evidence, for different contexts, also comparing with the guided standards proposed by the Order of Nurses. Finally, it can be stated that through the activities listed throughout the report it was possible to develop specific skills of the Nurse Specialist in Medical-Surgical Nursing, in the area of Nursing for Critical ill patient, namely within the scope of Infection Control and Resistance to Antimicrobials and also Complication Prevention.
