Abstract

This report is part of the Curricular Unit - Final Internship and Report - of the 1st semester of the 2nd year of the 16th Master's Degree in Nursing with a Specialisation in Medical-Surgical Nursing in the area of Nursing for People in Critical Situations, at the School of Nursing (Porto) of the Universidade Católica Portuguesa, in the 2023/2024 academic year, which includes two internship periods: the Multipurpose Emergency Service and the Cardiothoracic Intensive Care Unit. Its main objective is to describe the internship programme developed in the different contexts, based on critical and reasoned reflection, based on scientific evidence. It begins with a brief summary of the competences developed in the context of the Polyvalent Intensive Care Medicine Service, as part of the Curricular Unit - The Critically Ill Person and the Family: monitoring and clinical decision-making, which took place in the 1st year of the course. This is followed by a chapter characterising the two internship contexts developed within the scope of this Curricular Unit. The fourth chapter analyses and reflects on the common competences of the specialist nurse in the areas of professional, ethical and legal responsibility, continuous quality improvement, care management and the development of professional learning. This chapter presents a protocol for action in the event of activation of the Green Lane for Cerebral Vascular Accident, with the main aim of standardising action in the event of its activation, in the context of the Emergency Department. In the context of the Cardiothoracic Intensive Care Unit, the development of a proposal to include the Behavioural Pain Scale in the nursing record sheet was identified as an opportunity for improvement, for the assessment of pain in critically ill people in the context of the Intensive Care Unit, particularly in sedated and ventilated patients. The final chapter focuses on the specific competences of the specialist nurse, addressing the application of knowledge in professional practice through the pursuit of excellence in the provision of nursing care, developing skills and competences that make it possible to recognise and anticipate signs and symptoms of deterioration and instability in people in critical situations. The internships generated implications for professional practice, including preparation for dealing with a situation of haemodynamic instability and/or emergency, as these are situations that can be experienced in any professional context, including hospitalisation.
