Apical Periodontitis (AP) is an endodontic lesion produced by an immune response of the host, to the irritants products resulting from the necrosis and infection of dental pulp, and who persist in root canals, even after endodontic treatment. Recent studies have shown that some systemic conditions or behavioral habits termed "disease-modifying" may also affect the cure of AP. The tobacco influence in the prevalence of AP has also been studied by the fact that cause changes that lead to increased bone reabsorption and increased difficulty in healing process. Aims: To determine the prevalence of AP in a Portuguese population and understand the influence of tobacco in periapical status. Materials and Methods: Medical records and panoramic radiographs of 402 patients with at least 8 teeth in the mouth and more than 18 years old, who were treated or are undergoing treatment at the Dental Clinic of the Catholic University of Viseu, since January 2015 to March 2016, were evaluated. The registration of presence of AP associated with tobacco behavioral habit, per individual were recorded. The teeth were assessed according to the periapical status, presence and quality of endodontic treatment, presence and quality of coronal restorations, presence of cavities and root fragments. Results: Were evaluated a total of 402 patients and 9056 teeth. 57.2 % of the population represented the female gender and 42.8% male gender. The mean age was 46.81 ± 17.553 and the prevalence of smoking individuals was 23.9 %. The prevalence of teeth with AP (PAI≥3) was 4.7 % and 53.3% of the participants had at least one AP injury. The AP lesions shown to be related with the presence and quality of endodontic treatment, presence and quality of coronal restorations, root fragments and presence of cavities. The association values between smoking habits and number of lesions of AP were significantly related (p <0.05), however, the intensity ratio was moderately low (Cramer's V 16.2%). Conclusions: The number of lesions of AP was shown to be associated with smoking habits. The association with the most significant values was between the presence of AP and poorly endodontically treated teeth. More studies are needed to assess the effects of smoking in periapical lesions as well as all others risk factors.
