Nowadays’ policies contest the institutionalization of chronically ill psychiatric patients, whereas deinstitutionalization and rehabilitation becomes a priority, by changing the national model into a community centered intervention; the way of facing mental illness evolved through the years, from exclusion until a community based approach. In this context the Mental Health and Psychiatric Specialist Nurse has a decisive role in the rehabilitation process and is capacitated for the individual monitoring of patients, helping the patient/family to develop social skills and daily life skills towards autonomy and social inclusion. This work results as a priority in order to show evidence that it is possible to deinstitutionalize chronically ill psychiatric patients with quality and success, when there is a careful planning and a personalized monitoring. In this perspective a set of techniques was applied (aid relationship, active listening, memory training, problem solving, showing availability, social skills training, reality oriented therapy, reminiscence therapy) to two male patients over 65 years old, retired, earning a social pension, without family support, with psychotic pathology (schizophrenia), inpatients in UDEP for at least 25 years, regarding their rehabilitation and their deinstitutionalization processes. With this methodology, it was noted that patients have benefited of a better quality of care and life, thereby contributing for the development of a deinstitutionalization program running in UDEP, ensuring the continuity of care and life quality for the patients.
