Abstract

In developed countries, ageing populations are increasing, and consequently the prevalence of chronic diseases. Often, coupled with increasing age, coexists decreased independence for self-care activities. The current social Policies tend to maintain elderly people, as long as possible in your home, even with moderate to high dependency, becoming a challenge for health professionals, for lack of human resources, and to informal caregiversthat require cognitive skills and learning abilities. The role of care provider, played in many cases, an element of the household, becomes an asset to continuity of care. But, sometimes, caring for someone with dependency, you can change the family dynamics. Was due to a perception of health professionals of Acute Family Health Unit, to a concern of students of internship (workgroup) and literature review, which emerged a health Diagnosis to family group (families in Acute family health Unit)whose household included a chronic carrier of pathology in the thematic approach of family dynamics, dynamic template Family assessment and Intervention (Figueiredo, 2009). After data collection, processing and analysis, it was possible to establish the needs of families included in the diagnosis of health, executed during the module I Stage. Through prioritization by the technique of pair-wise comparison, problems, (translated into outbreaks of nursing) that should be the subject of Community intervention. In module II Stage, it was proposed; develop to respond to at least one of problems found. Focus Healthcare Provider Role, with appropriate judgment, was not selected for the project called Family Care, which had , aims to sensitize the community to the role of the family in informal foster care, the importance of the role of care provider within the family and in the multidisciplinary teampromote the role of provider care in families and provide formal/informal training families on the role of care provider. The achievement of two Stage modules, provided the development of more professional skills, going to meet the recommended by syllabus of the masters course with specialization in the area of Community Nursing and regulation of specific competencies of Nurse Specialist in Community Nursing.
