This report represents the result of the master`s degree “Curso de Mestrado em Enfermagem área de Especialização em Enfermagem Médico-Cirurgica” granted by the Universidade Católica Portuguesa, Instituto de Ciências da Saúde, Escola Superior Politécnica de Saúde, in Lisbon. The present study was suggested and proposed during the medical-surgical nursing traineeships, being related to all the stages performed throughout the course. This master`s degree includes three stages: emergency department, intensive care unit and na option to pick from the following áreas: paliative cares, hospital infection or operating theatre. Considering that my work assignment has been at the operating theatre of Hospital Professor Doutor Fernando Fonseca EPE for six years, i was granted formal crediantials and recognized competence not to take the third stage, according to the portuguese act number 74/2006 in force since the 24th of April of 2006. While at the Hospital Professor Doutor Fernando Fonseca EPE operating theatre, it was my duty to provide health care to medium and high risk patients at the nursing perioperatory área. At the moment, i coordinate the training team care management and also supervise the nursing support for the Maxilofacial surgery speciality. In the relation to the other two stages, my training was carried out at the Hospital de Cascais Dr. José de Almeida`s Emergency Department and also at the Hospital Professor Doutor Fernando Fonseca EPE intensive cardiac care`s unit. Despite my two years` experience in intensive care and other four years at the general emergency service in a central hospital, i believe that the contínuos learning, practice, aknowledgment of new techniques and theories are essencial along the entire nursing profession. Today, every nurse care and act must be based on solid knowledge. Portuguese Ordem dos Enfermeiros believes that a specialized nurse in patients in a critical situation is «the one who moves knowledge and multiple skills, responding in time in a holistic manner to complexity of health circunstances, to the person in a critical disease situation and/or organic failure and his family». Nursing care provided by a specialist nurse to a patient in critical situation due to natural disasters and other catastrophes, accusing multi-organ failure in a palliative phase of the disease, require much from the nurse in service: observation, contínuos evaluation and immediate action in order to keep vital functions, preventing complications and limiting disabilities.
