This internship was developed in three modules, being that the first two were undertaken in the health area and the third in a community intervention project of a city hall. The school intervention took place in the south-west ACES, where it seemed there were no children with health care needs. The study conducted identified in one of the clusters of schools 96 children with varied health necessities, being the pathologies of the respiratory system the most prevalent and asthma the most significant. While studying children with asthma, one found that 18,5% are not being followed by speciality consultation and therefore are those that most resort to the emergency service; none have any individual health plan instituted and all children are daily in the presence of many precipitating factors of asthma, whether they are at school or in a domestic environment. Faced with this situation diagnosis, there were designed a set of activities aimed at addressing this problem. Therefore, the I and II modules had as main objectives: to promote the routing of asthmatic children that are not followed in speciality consultation; to establish individual health plans for the children with asthma; to promote the acquisition of knowledge and capacities about asthma to the local community and to intervene upon the environmental risks of asthma in a school context. To achieve these objectives our attention was based on directing children to a follow-up speciality consultation, domiciliary visits were carried out, explanatory sessions about asthma were promoted and a manual of “Good Practices of Hygiene in School Environment” was created. The III module of the internship was developed at the city hall of Peniche and integrated in the GPS - Management of Proximity for Sustainability. This project was processed from the needs felt by the multidisciplinary team, in order to provide an effective response to an existing problem in the neighbourhoods of social housing in the city of Peniche. From this problematic, it stands out that 20% of the studied households are constituted by elements with a dependency in daily living activities and consumption of illicit substances; almost 50% are constituted by elements with >18 years of age that do not have an adequate level of schooling for their age; 40% are in an unemployed situation and 74% have monthly income per-capita(monthly) < 189,52€. In view of all these results, an intervention was conducted upon a family’s problematic, resorting to the use of the Dynamic Model of Evaluation and Family Intervention and a nursing care plan was established and directed to the family, based upon nursing diagnostics that faced the identified necessities.
