Abstract

The present document reports a project’s execution to intervene in the person, family and community, regarding each one of the levels of prevention, with the main issue, the abuse and addiction of psycho-active substances. The interventions elapsed individually and in group, using as a methodology, the motivation for behaviour change seeking healthy life styles, regarding the transtheoretical model of Prochaska and DiClemente and the Relapse Prevention according to Marlatt’s Model and the helping relation as an therapeutical instrument. Amongst other interventions, we have had the Education for Health sessions and a Relapse Prevention program. In order to optimize the Nursing Process and records, vocational education and training sessions concerning the International Classification for Nursing Pratice® were carried out. The implementation was side by side with the person, held responsible for it's health, seeking health behaviours, the abstinence of consumption and the relapse prevention. The approach aimed the specific know-how and skills of the nurse specialist in Mental Health and Psychiatric Nursing emphasizing the important role of these elements in health teams.
