This report aims to be a descriptive and reflective analysis of the skills developed during the clinical practice in medical-surgical nursing specialization course, as well as an assessment document for this course. The report structure is divided into three main chapters: the first is the introduction of the report, with the theoretical background that formed the basis of my intervention; the second part includes the description, analysis and reflection of the goals and activities developed in clinical practice and skills acquired; Finally a brief conclusion is made to show how this experience has promoted my development as a nurse and as a person. In Module I - Emergency Department, I joined the staff at the emergency department at Santa Maria Hospital; In Module II - intensive care unit, was performed in the intensive care unit for infectious diseases at Santa Maria Hospital; Module III was held at the home care unit of Instituto Português de Oncologia, in Lisbon. During the clinical practice I had the opportunity to develop skills in the area of providing patient care and other more complex situations and family care. The nursing care started from an advanced nursing practice, based on scientific evidence and followed the care of the technical point of view, the management of special care, in critical consciousness to the problems of practice, but also based on a therapeutic relationship and development of communication processes. For the care of family / caregiver/people I have based my intervention on the theory of transitions of Afaf Meleis, on the premise that the family throughout the life cycle through periods of crisis that require a further reorganization and finding equilibrium points for effective adaptation. The nurse's role in the experience of transactional processes is to recognize the moments of crisis and identify resources available in the family to facilitate the health/illness transition processes. The skills acquired at these different levels and the thought that came out to allow construction of a professional identity as a nurse specialist in medical-surgical nursing.
