This report is part of the evaluation in the syllabus of the 7th Master in Nursing with specialization in Community Nursing, Institute of Health Sciences of the Portuguese Catholic University. It integrates the exposure of paid route, resulting in phases the set of all activities performed. Perception, analysis and action thus form a common thread that justifies the strategies highlighted and used to address the problems that arose. Thus, the module I corresponds to Health Diagnostic phase where a data collection instrument was developed and then applied to a population selected by convenience in the scope of the Community Care Unit (CCU) of Campanhã. This first phase defined as areas of intervention Management and Adherence to Medication Regime. The Community Intervention refers to the module II where the four target users of this intervention were referred by UCC campaign is randomly distributed and at the end it was possible to say that in all users covered by the intervention were obtained health gains. Finally the III module, optional character, developed in the Team Continuous Care (ECCI) Campanhã and Personalized Health Care Unit (UCSP) of São Roque da Lameira in order to develop knowledge and skills in monitoring and the individual's assistance in their life processes, culminating in the development of a standard for UCC / ECCI campaign on the Surface Support. The methodology set out passes essentially the exploitation of statistical evidence resulting from the forms made with the population considered. After a characterization of the surveyed a random sample, the statistical processing has highlighted nonparametric trends and correlations considered relevant and explained in this document In a global context and to finish, a critical reflection approaches and summarizes the strokes and their outcomes objectives, these difficulties and skills developed in the course Specialists in Community Nursing concerns.
