Abstract

This report tells the process developed during the traineeship, in order to acquire skills for the specialization in Medical Surgical Nursing within the Master´s in Nursing. The selected methodology was the descriptive and analytical method, done through reflection and description of activities and it is structured in four areas of competence. The traineeship included three modules carried out in the emergency of Pedro Hispano Hospital_Module I, in the Unit Cardio-Thoracic Surgery of São João Hospital_ Module II and in the Norte Regional Branch of the Nacional Institute of Medical Emergency _ Module III that occurred between 23/4/2012 and 2/2/2013. After having contacted with different realities, patient and family, it became important to reflect on ethical considerations, aspects of communication / family and the importance of their involvement in the caring, aiming to humanize it, services leadership and management instruments and the role of the nurse specialized in Medical-Surgical Nursing. Furthermore, it is also important the relevance of the registration nursing in the continuity of care, the monitoring of pain and the therapeutic procedures for its control, the importance of the execution rules that allow the standardization of attitudes and ways towards care related to the professionals, namely concerning the greenways of stroke/trauma and about the inherent specific care of cardiac transplant patients. It was also given relevance to the area of infection associated with health care, particularly to the Pneumonia Acquired through Ventilator as an important action area of the specialized medical surgical nurse. The action in the different contexts of traineeship has been headed for the necessities identified and considered as relevant by the services. In this way, were approached, in action training, the attitudes in caring for the patients, in agony, in the emergency, which allowed to understand the particularities inherent to death and palliative care, and the announcement of bad news in context pre-hospital, leading to a consciousness and awareness about details concerning their communication, as well as to verify that the nurse in Medical Surgery service should not only acquire technical knowledge, but also the one related to communication. It was broached, in the module III of the traineeship, the accession to the therapeutic process, recognized and suggested as a formative necessity, after the registration of the activations achieved, being one of the subjects that most calls attention among professionals, leading to the nurses’ reflection. The approaching of these issues activated pertinent features, providing moments of introspection about the nursing practice, perceived and provided, and helped in the perception of restrictions. The period of traineeship allowed, not only to know the emergency acting area of pre-hospital, where unpredictability reigns, but also to identify a different reality regarding Intensive Care, instigating to the rethink and reflection about practice and the way of acting in this area.
