A report aims to point out a set of information about an individual experience, design, research, or other activity for the purpose of reporting what was done and what was left to do. In this context, considering that this explanation is the corollary of the course unit Relatório, it is attempted to expound the traineeship developed in Emergency Room (Module I) and Polyvalent Intensive Care Unit (Module II), which took place in Centro Hospitalar Vila Nova de Gaia/Espinho. In essence it is intended to be emphasized the development of skills, based on the goals initially set, in the domain of professional, etical and legal responsability; on the continuous improvement of quality and on the management of care and professional learning processes. This report scopes out the following objectives: to show the acquired skills, the identified problems and its solutions; to demonstrate critical-reflective thinking about the actions performed and the consequent ethical and social implications and finally to display the ability to communicate properly the conclusions. In the first module it was possible to develop activities providing the care to critically ill patients in the Emergency Room with a large amount of users with a physical incompatible structure with a reduced number of nurses in some areas. It is a service that requires a considerable level of management knowledge as it has a wide range of human resources, materials, equipment and constant interactions which modify the internal homeostasis. Thus, appart from the daily experiences on the Emergency Room I could contribute, through the training session, to the processes of leadership, motivation and strategies for change, involving the team coordinators and sub-coordinators, as well as the leaders in charge for the service. With a deep sense of reflective spirit it was an initiative that sought to bring together the key elements for change. In the second module, the Polyvalent Intensive Care Unit, skills in providing care to critically ill and/or multiorgan failure patients were developed. It is a technologically complex environment in permanently challenging taking decision processes associated with a higher autonomy level of the nurses. Therefore, it implies a more strict clinical judgement. In this module with different features from the previous, it was possible to add scientific knowledge to nursing, an area of great autonomy, particularly in the prevention of pressure ulcers. So, I could carry out a retrospective epidemiological study investigating the prevalence and incidence of pressure ulcers in the period from 31st October 2012 to 31st October 2013. The results were encouraging because, although the number of hospitalized patients has increased significantly since 2007 (most recent data), there was a decrease by half the incidence of pressure ulcers. In this report it was used a descriptive methodology using the critical reflective thinking. It is divided into two parts, in the first it is performed the characterization of Centro Hospitalar Vila Nova de Gaia/Espinho, the ER and critical care. In the second, it is described the construction of skills, within the completion of activities in the four recommended areas for the specialist nurse.
