Abstract

This document is a probationary report entitled “A SWOT Approach in the nursing Community Framework” and describes the skills acquired throughout the training process of the Masters Course in Nursing with Specialization in Community Nursing, with a particular emphasis on the skills developed during the Community Nursing training. The training period had three sequential modules conducted in the Care Unit of Senhora da Hora, which is integrated in the Local Health Unit of Matosinhos. Module I on the community health diagnosis aimed at developing a diagnosis of the health condition and identifying needs on nursing care; For data collection was carried out a form, whose sample consisted of clients with a diagnosis of diabetes mellitus enrolled in private clinics of Senhora da Hora. The conclusions of diagnosis were grouped into key groups in order to enable the setting of priorities, as reflected in: inadequate monitoring of blood glucose, impaired Adherence to medication regimen, health-seeking behavior compromised, inadequate monitoring of the feet, Accession committed to the dietary regimen, use of community resources and sources of information ineffective, Adherence to exercise regimen compromised, pathophysiologic changes associated with DM. In Module II was an Intervention Project Carried October Focused on the needs Identified; giving relief to be adherence to treatment regimen. And Module III (optional) organized several activities related with health quality versus health gains. SWOT (Strengths, Weaknesses, Opportunities and Threats) analysis was systematically used to assess the various scenarios within the community framework and to introduce the activities performed. In writing this report, we followed a descriptive methodology based on the knowledge acquired during the initial training, postgraduate training, professional experience, as well as on bibliographic research, in order to challenge it by scientific evidence. The skills on Community Nursing have been systematically developed, namely the accomplishment of a Health Planning (diagnosis, intervention and evaluation), the deepening of communication techniques within a community framework and the completion of some proposals of intervention on Health Quality.
