This report aims to present the work, the skills acquired and some of the results obtained during the stage of Modules I and II, of the Master Course in Community Nursing. Diabetes is a chronic disease that is emerging as a worldwide problem, affecting millions of human beings, involving high social and economic costs. Its incidence in Portugal has been increasing since the year 2000. The prevalence also continues to grow and it is associated with several factors, such as the aging of the population, the dietary changes, the unhealthy lifestyles and other unhealthy behaviour patterns. In this context, it is urgent to act at the level of Health Promotion in the community and empower patients to change their habits to healthy living habits with better life quality. The diabetic foot is one of the most serious complications that results from diabetes, which can lead to amputation Minor and Major, who besides burden the economic costs of the country, also causes changes in personal and family lives. This requires that we act more focused and efficiently to the level of Primary Prevention, so there remains the need to encourage health providers to not bleach the surveillance of diabetic foot complications to delay possible complications. The planning and intervention of community nursing actions, took place during the stage at the S. João Evangelista dos Lóios, owned by ARS Lisboa e Vale do Tejo, IP, embedded in ACES Lisboa Oriental II. Initially, we performed a situation diagnosis of the community, to frame and contextualize the reality where diabetic users were inserted. The analysis of the diagnostic situation, guided the intervention for care at primary and secondary levels. In this context, various activities were carried out involving several "actors", from the local community and health professionals, always bearing in mind the Promotion and Education for Health. Stands out among these activities, the implementation of the Nursing Consultation for Diabetic Foot and yet the realization of queries of Nursing, Education group sessions, Street Activity, Elaboration of a Protocol for the implementation of Nursing Consultation. The results obtained in the course of the activities carried out had a huge impact and acceptability by patients and health professionals
