This paper consists in the description and analysis of activities undertaken during Training, within the Master of Nursing (specialization in Community Nursing), held at Unit of Community Care in Senhora da Hora. Given the methodology of health planning, was done a situation analysis on the focus of nursing: management of therapeutic regimen in Diabetes. The target were 81 diabetic population living in Custóias, Private Clinics belonging to I, II, III, in agreement with the National Health Service. After the definition of priorities emerged as priority areas of intervention, the components of diet and physical activity, proceeding to the implementation of three community intervention projects. 31% of the initial target population, was the subject of the projects "Healthy eating in Diabetes" and "Diabetes on the move". The "Healthy Eating in Diabetes" was aimed at giving diabetics the level of knowledge of the diet component by conducting educational activities for health. The outcome evaluation was conducted using a questionnaire administered at the beginning and end of project implementation. The targets were exceeded and the objectives achieved, and there are 43% gain in knowledge. The "Diabetes on the move‖ aimed to give diabetics the level of knowledge of the physical exercise component, by carrying out activities in health education theory and practice. The outcome evaluation was conducted using a questionnaire administered at the beginning and end of project implementation. The goal related to the number of activities on health education was not achieved due to unfavorable weather conditions. However it is considered that the overall goal was reached, and there are 56% gain in knowledge. The group's assessment of satisfaction in the projects was done through questionnaires, it was found a figure of 100% of satisfied patients. Overall the structural assessment identified, the cold as unfavorable factor associated with inadequate facilities where it took place in health education. The commemorative session of the World Diabetes Day aims to inform the community of Senhora da Hora on the prevention and treatment of Diabetes, and promote the adoption of healthy lifestyles. The targets set were exceeded. Present at the event about 190 people. The form of assessment of risk of developing diabetes was applied to 82% of the population screened. Surpassed the target of 60% established, having been identified 45 people with risk ―significantly elevated‖ to develop the disease. The general objectives of projects are achieved by verifying a suitability of the strategies and activities implemented.
