Abstract

Introduction. This study consists of an umbrella review, which synthesizes the evidence described in scientific literature on the resistance of different Candida species, to antifungal agents, highlighting the clinical impact on vulnerable populations such as immunocompromised patients (diabetes, HIV, cancer). Materials and Methods. A search was conducted in PubMed, Scopus, and Web of Science databases, to identify systematic reviews and meta-analyses, published between 2000 and 2024. The studies included were selected based on inclusion/exclusion criteria, and a risk of bias assessment was performed using the ROBVIS tool. Results. The results indicate a worrying increase in the resistance of non-albicans species to antifungal agents, particularly resistance to azoles, such as fluconazole. Candida auris was found to be particularly resistant to both fluconazole and voriconazole. Comorbidities such as HIV and diabetes are associated with a higher vulnerability to infections by Candida spp. Conclusion. This study highlights the need for the development of new anti-fungal treatments, as well as a more personalized therapeutic approach. Strategies for monitoring anti-fungal resistance are essential to address this growing challenge.
