Abstract

Demographic ageing and changes in the epidemiological pattern and in social and family structure of portuguese society, have determined new needs in health, for which it is urgent to organize appropriate answers. Although enormous progress of health sciences, in recent decades, have had a major role in increasing longevity, the portuguese reality shows that the last years of life are frequently accompanied by situations of fragility and incapacity often related to health. The “National Programme for the Health of the Elderly”, aiming to maintain autonomy, quality of life and overall recovery of the elderly, primarily in domicile and habitual way of life, requires a multidisciplinary action of health services, in close liaison with the “Network of Health Continuing Care” (DGS, 2004). The application of an integrated conceptual model, embodied in the “Network of Continuing Care”, will have more success, the more it is based on cooperation, through the establishment of partnerships that create synergies between experiences, skills and resources among the different health institutions and with respect for ethical principles of transparency and mutual understanding. This report describes the implementation of a community intervention project, sustained in lifting the needs expressed by the continuing care staff of the Health Centre of Oeiras which ran under the first stage module. The second module was based on the implementation of strategies directed to the articulation and development of a partnership between the care of the Group of Health Centers of Oeiras and Egas Moniz Hospital in nursing perspective. The third and final module has developed the extension of this draft to other teams of continuing care and other residential services in the coverage area of Egas Moniz Hospital. With this project called "Network Care", one intends to promote the development of partnerships in order to contribute to the promotion of active ageing and more healthy elderly population through the continuation of nursing care.
