The human papillomavirus is one of the most common sexually transmitted infections worldwide. The National Vaccination Program includes the vaccine against this virus. Its vaccination scheme encompasses the adolescent age, a period of development that drives the adoption of risk behaviours. Thus, it becomes imperative to obtain an effective immune response. Health policies advocate improvements in the health of the population and nurses in functional health units’ play an important role in the implementation of intervention projects. The Health Promotion Model by Nola Pender and the Health Planning Methodology by Imperatori and Giraldes constituted a grounded guideline for decision making and the achievement of results. The theoretical foundation was based on a literature review. The diagnosis of the situation was established with a focus on the problem of low rates of vaccine coverage against the human papilloma virus in adolescents, in the 1st semester of 2021 of an ACES, followed by interviews with experts in this field of action of two health units. With the analysis grid were prioritized the nursing diagnoses resulted of the interviews. Subsequently, objectives were set and organizational and educational strategies were established through the elaboration of a scoping review. The + Saúde para Todos project emerged, included in primary prevention, and aims to achieve health gains through compliance with the vaccination coverage rates of the National Vaccination Program of the cohorts elected by the Directorate-General for Health. Given the time limiting factor of internship, inter-institutional partnerships were established and essential educational materials were created to optimize the response to health services and to train adolescents in the adoption of health-promoting behaviours related to vaccination against the human papilloma virus. Despite the difficulty experienced in carrying out the activities, the interest shown by the inter-institutional partners was evident and essential for the continuity of the nursing intervention project and in the future with the extension to other vaccines and cohorts elected by the Directorate-General for Health. The entire journey was crucial for the development of personal and professional skills common to the specialist nurse and specific to the specialist nurse in community health and public health and master's nursing.
