In healthcare providing, continuous professional development, as advocated by many authors, takes a crucial role and allows substantial gains. The continuous improvement of the care provided, associated with the change of practices is clearly one of those gains. This report aims to describe, analyse and focus on the inherent reflection over specialized care nursing in an internship context. This internship took place in three distinct clinical wards, in which the approach of the person in critical condition/family and the nurse's decision-making were clear, revealing an extensive role throughout this process of skill achievement. Considering the goals and competencies outlined for this training path, strategies, activities and interventions implementation allowed the acquisition, consolidation, mobilization and application of solid knowledge that a specialist nurse in medical surgical nursing should have well in mind when providing care. The compelling need to use research as the support base for nursing practice – practice based on the experienced situations and opportunities provided at the Emergency Room (ER), Operating Room (OR) and Intensive Care Unit (ICU) - lead to elaborating a systematic revision of the literature based on the nurse’s decision-making when approaching people in critical condition. The sharing of knowledge, the stimulus to acquire new capabilities and the reflection on and about the practice are challenges to improving nursing care.
