Introduction: Recognizing the needs of palliative patients is a crucial challenge in Palliative Care. Tools like the Support Needs Approach for Patients (SNAP Tool) have shown a positive impact on communication between the patient and healthcare professionals, helping to identify patient needs. The aim of this study is to translate and validate the SNAP Tool for the Portuguese population. Methodology: The initial phase of the study focused on the translation and cultural adaptation of the tool to Portuguese, followed by a second phase where the understanding of the pre-test version was validated through semi-structured interviews with patients. The study involved a sample of nine participants, consisting of palliative patients in their home care context. Results: The results indicate that the translated version of the SNAP Tool, like the original version, allows patients with palliative care needs to express their needs to their healthcare team. Conclusions: The purpose of this study was to validate the SNAP Tool for the Portuguese population, ensuring that it is understood and relevant for patients with palliative care needs in Portugal. The SNAP Tool aims to facilitate communication between the patient and the healthcare professional, helping to identify patient needs and enabling a more personalized care plan. The Portuguese version of the SNAP Tool has the potential to improve the identification of needs and, consequently, the quality of care provided to patients with life-limiting chronic diseases.
