Introduction: The report summarizes the internship performed in a Community Care Unit (CCU) in Setubal during September 3 and December 19, 2018. The internship has been done as part of the Master in Nursing with specialization in Community Nursing. Methodology: According to the objectives defined for the internship, the project “High Users of Emergency Services: specialized intervention in the Family Health Teams” has been developed. The project has been based on the Health Planning Cycle by Imperatori & Giraldes (1), according to Betty Neuman Systems Model (6) . To perform the diagnosis of the health situation, a detailed description of the CCU, the general group of High Users and the High Users from the regional area of the CCU has been done. To perform the detailed description, a quantitative analysis was performed, using retroactive data (August 2017 to August 2018) of the usage of the Emergency Department (ED) of the hospital of the same regional area of the CCU. In addition, an interview was conducted to the Nurse responsible for coordination of the CCU and questionnaires were applied to coordinators doctors and nurses of each functional unit identified, and to family´s doctors and nurses of the patients identified as high users of ED that also had interventions of the High Users Resolution Group (HURG) in the same functional areas. The data has been analyzed using quantitative and qualitative methods. Results: The following nursing diagnosis were identified: deficiency of knowledge of the structure of the HURG due to lack of disclosure of this information and poor coordination between the Family Health Teams and the HURG considering that the procedures are not standardized. For each diagnosis an action has been defined, including definition of priorities, objectives and strategies, to guarantee that the project supports the fix of the problems identified. Discussion: The activities defined were executed, as a result the key performance indicator defined for the project were mainly achieved or exceeded. The ethical questions during the internship were always considered, that is explained through the report. The internship experience allowed for the development of several competences: general competences of Specialized Nursing (3), specific competences of Specialized Nursing (4) and Master (5) . Conclusion: The objectives defined to obtain the Master and Specialized Degree in Community Nursing have been accomplished by the student. The achievement has been done through the development of a community intervention project for the targeted population, that will result in better health, according to the priorities of the current Health National Service.
